professionals will also service

Premier
Transitional Care
Program is
a licensed, independent
facility that provides
a safe short-term
living environment.
It also supports
intervention and
motivational services
for at-risk youth.
The mission of AXIS Foundation, Inc. is to unlock the potential of
at-risk young people of southern California
in order to build and sustain healthy communities.

AXIS
Foundation's staff and contractors
have been community advocates,
and support programs that provide
our youth additional safety
nets, with partnerships that
include gang intervention organizations,
the LA County Probations office,
and Transitional care homes.
We
are currently developing
a pilot program
with the Centinela
branch of the Los
Angeles Probations
Department.

Management,
we are

with
